WHAT IS A CLITORAL HOOD REDUCTION?

Clitoral hood reduction, also known as a clitoral hoodoplasty, is a surgical procedure in which extra tissues around the clitoris are trimmed or removed. The purpose of this procedure may be to reduce the size of the clitoral hood to improve appearance, or to expose more of the clitoris to improve sexual stimulation.  The procedure can be performed in the office under local anesthesia or with deeper anesthesia when performed with other vaginal procedures.

WHAT IS THE CLITORAL HOOD?

The clitoral hood is defined as the folds of skin surrounding and covering the glans clitoris.  Because the clitoris’ sole function is to provide sexual stimulation, it is highly sensitive and contains thousands of nerve endings.  The primary function of the clitoral hood is to protect the highly sensitive clitoral tissue and is the female version of the foreskin surrounding the glans penis in males..  Among different women, the clitoral hood can vary greatly in appearance, ranging from thin and petite to large and redundant. There is no right or wrong appearance of the clitoral hood, but there are certainly aesthetic preferences in how it appears both by itself or in relation to the other vulvar or vaginal tissues.

AM I CANDIDATE FOR CLITORAL HOOD REDUCTION?

If you are unhappy with the size and shape of the skin around your clitoris or if the excess clitoral hood tissue is interfering with your ability to achieve orgasm or sexual stimulation, you may be a candidate for a this procedure.  Some women with excessive clitoral hood tissue may find wearing certain types of tight clothing uncomfortable or embarrassing. Another reason to consider clitoral hood reduction is if you are undergoing a labiaplasty to reduce the size of the labia minora, which may result in an “unbalanced” appearance to the vaginal lips if the clitoral hood is not reduced simultaneously.

HOW IS CLITORAL HOOD REDUCTION SURGERY PERFORMED?​

Surgical clitoral hood reduction can be performed under local or general anesthesia.  During the procedure, Dr. Kafali will first mark and anesthetize the area. Once the area is numbed, the excess clitoral hood skin is then carefully resected using a scalpel or laser.  The resection can either be done on the lateral clitoral hood tissues in a diamond shape pattern, in the central area above the clitoris itself in an upside-down moon shape, or in both areas in an upside-down horseshoe shape.  Bleeding is carefully controlled and the incisions are generally closed using self-dissolving sutures. The type of excision and the amount of tissue removed will be determined by you and Dr. Kafali depending on your aesthetic and functional goals.

WILL CLITORAL HOOD REDUCTION LEAVE SCARS?

As with any surgical incision, there will be scars.  However, the skin around the clitoral hood is very thin and flexible with a good blood supply, so generally heals very well.  After a few months, scars in this area are almost invisible and are usually not noticeable or obvious to the naked eye.

HOW LONG DOES CLITORAL HOOD REDUCTION TAKE?​

Clitoral hood reduction can typically be performed in less than one hour.  However, clitoral hood reduction is often done in conjunction with other procedures like labiaplasty or labia minora reduction which can extend the procedure to a longer duration.

WHAT IS RECOVERY LIKE FOR CLITORAL HOOD REDUCTION?

Clitoral hood reduction should require little to no down-time for recovery.  Immediately after the procedure you should expect some mild bleeding and some mild to moderate discomfort once the anesthetic wears off.  You will be instruction to take it easy for a few days and avoid strenuous exercise. As with other labial or vaginal procedures, you will be asked to abstain from sexual activity for up to 6-weeks to allow the tissues to heal properly.  Most patients can return to work in 1-2 days and can resume exercise in 2-3 weeks after clitoral hood reduction.

DOES CLITORAL HOOD REDUCTION HURT?

As with any surgical procedure, you can expect some discomfort following clitoral hood reduction.  However, most patients find the discomfort tolerable and are easily treated with ice and over-the-counter pain medication like Tylenol.   Although uncommon, any excessive amount of pain or swelling should be brought to the attention of your surgeon immediately.

WHAT ARE THE RISKS OF CLITORAL HOOD REDUCTION?

The risks of clitoral hood reduction are minimal and serious complications are exceedingly rare.  Bleeding, swelling, and bruising are generally minimal and should resolve by 2-3 weeks. The clitoris itself has many nerve endings and although very uncommon, nerve injury to the clitoris can occur and, in some rare cases, can be permanent.  If the area around the clitoris becomes overly swollen, it can cause a urinary blockage and will need to be addressed immediately. Overall, when clitoral hood reduction is a very low-risk and safe procedure when performed by an experienced surgeon.
